Paperwork

Two clearances are needed, and possibly a third if you have not been a resident of PA for 10 consecutive years. 1. PA State Police Criminal Background Check – Completed online and renewed every 5 years. Click here to begin. 2. PA Child Abuse History Clearance – Completed online and renewed every 5 years. Click here to begin. 3. FBI Criminal History Record – This check is only required if you have lived in PA for LESS than 10 consecutive years. Completed partially online and then fingerprinting must be completed at a fingerprinting location. Click here to begin. Use Service Code 1KG6ZJ when registering.

All completed forms can be emailed to soccerfun@comcast.net.

U5 - U8 Coaches are required to watch a short coaching introduction video called “Introduction to Grassroots Coaching” on the US Soccer Learning Center’s website. You will need to register in order to access this short course.

You will also need to complete a PA Child Protective Services Law Disclosure Statement for Volunteers and a PMSC Coaches Code of Conduct form each Fall and Spring season.

You also be required to complete the “HEADS UP to Youth Sports” online training video each season (incorporates the Fall and Spring season cycle).

You will also need to complete the US Soccer Connect’s Abuse Prevention course located on their website.
